ENT New Patient Information

Scheduling an Appointment

Please contact our office to make an appointment. If you or your child are a new patient to the office we ask that you bring the following:

  • Any x-rays, CTs or MRIs that relate to your condition
  • Complete list of medications
  • Any other information that may be of assistance during your visit.

For the clinic near you, please call the number listed with that clinic.

Telephone Information

When you call the clinic for health care information, our specially trained staff will assist you. A nurse or physician will return your call as soon as possible. To help assist us when you call please provide us with a phone number of your pharmacy.

Insurance and Payment

We participate in many insurance plans. Please refer to your insurance provider directory for a complete list of participating physicians. If your insurance is a co-payment plan or it is one in which we do not participate, we ask that you pay at the time of service. Our staff will assist you with the information you need to file your insurance. If your insurance plan requires you to have a referral to visit us, please remember that this is your responsibility to obtain before your scheduled appointment. Your primary physician can provide the referral.

Fees

Fees for office visits vary, depending on the level of care you receive and the time required for evaluation and treatment. Audiology and laboratory services are additional charges. Your charge is based on the type of service that our clinic staff provides.

You may pay with cash, check, MasterCard, Visa or Discover Card.
