What to Expect at Your First Physical Therapy Appointment

• Please arrive at your appointment 20 minutes early to fill out the patient questionnaire and insurance forms.
• (These can also be downloaded from this website under "Forms to Bring With You" below) 
• Please bring your insurance card and a photo ID.
• Please bring your physician’s prescription for physical therapy or occupational therapy or you may have it faxed to the appropriate clinic prior to your appointment.
• Please bring or wear clothing that can easily allow evaluation of the affected body part (such as shorts or tank top).
• Plan on approximately 1 hour for your initial evaluation once your paperwork is complete.

Billing

• We are participating providers with all major insurance carriers.
• We bill as an office visit
• We accept all major insurances
• We are Medicare friendly
