United States Immigration

· The following physicians are authorized as Civil Surgeons to complete immigration physicals:

·       Please bring with you:
      Your passport or I94 form
      A photo ID
      A record of all immunizations or vaccines you have received
      Immigration forms. (Please complete part 1 and then print. You
          can get them from this site:
          http://www.uscis.gov/files/form/I- 693.pdf

Canadian Immigration

· We are authorized by the Canadian government to complete Canadian immigration physicals:

o      Please bring with you:
     Your passport or I94 form
      A photo ID
      5 passport sized pictures
      Immigration forms. (Please contact the Canadian Immigration Office and these will be sent to you. This is not a form you can obtain from our office. It needs to have the seal of the Canadian government on the form).
